I am on H1B(4th year), have not initiated GC process yet. I need to take a break for about a year and travel to India for medical reason. After the break, will my visa be valid to get back to work?

I would suggest to contact immigration attorney to get first hand information.

There are so many attorneys available and they charge around $200. Don’t worry about $200 but you will have peace of mind.

Try Avvo charges around 100 for a 15 minute consultation.

Based on your case, from my personal experience, Yes you can take a break. Your time outside USA can be recaptured when you file your last extension. You get a total of 6 year H1b at a time without worrying about lottery. When you come back you can refile your H1b as CAP exempt (i.e. exempt from annual H1b limit & hence lottery).
